Reimbursement Application Packet
Applications for reimbursement may now be submitted to DATCP by email. Please make be sure all electronic application documents (i.e. forms, invoices, and receipts) are in the correct order prior to scanning and emailing. If electronic documents are submitted, then paper copies no longer need to be sent to DATCP. Please contact us with questions.
