Owner Hauler Statement for Lactating Dairy Cow Transport to Slaughter

​​On April 24, 2024, the U.S. Department of Agriculture (USDA) issued a Federal Order designed to reduce the risk of HPAI (H5N1) spreading among cattle and poultry. Additional guidance from the USDA on the movement of lactating dairy cows to slaughter was provided April 26, 2024. Both are in effect now and require:

“Nonclinical lactating dairy cattle moving interstate direct to slaughter are not required to have a premovement test but must move on a certificate of veterinary inspection or other documentation of movement approved by the sending and receiving state animal health officials and provided to the sending and receiving state animal health officials."

To facilitate these time-sensitive movements, DATCP has created a web-based form that, when completed by the animal owner, producer, hauler, or market representative, sends a copy of the form to participating states. States not participating are not included in the drop-down menu when completing the form.​

This Slaughter Movement fillable pdf form is also available for use. If using this fillable pdf form, you must also email copies of the completed form to both the state of origin and the state of destination. The email addresses for the states are available using the drop downs at the top of the form.

These forms are
Owner Hauler Statements and may be used in place of a Certificate of Veterinary Inspection (CVI) for moving healthy, lactating dairy cattle to slaughter in Wisconsin from participating US states. They may also be used to move healthy, lactating dairy cattle to slaughter from Wisconsin to other US states if accepted by those states. This form is not acceptable for international movements or movements other than directly to a slaughter facility. This form is not acceptable for movement across state lines to “slaughter only" sales, including those at federally approved facilities – those movements now require a CVI, negative Influenza A test within 7 days of movement, and official ID eartags.


*Please Note* Not all states accept these forms for moving cows to slaughter across state lines. Currently, these forms are not available for use for moving lactating dairy cows to slaughter in New Jersey, Ohio, Oklahoma, Texas, or Vermont (as of 5/1/24). It is the responsibility of the form user to contact the State Animal Health office in the state of destination for import requirements. Individual facilities may have more specific documentation requirements. Requirements are subject to change.​
