Plant Protection and Quarantine Form 540
PPQ Form 530 (limited permit)
Signed "accurate statement" (for logs, pulpwood and wood chips)
Digital stamp certificate (for Christmas trees and nursery stock)
Qualified certified applicator document (for mobile homes and outdoor articles)
Compliance Agreements
If businesses ship regulated articles out of quarantined areas occasionally, a state or federal inspector will inspect and certify individual shipments. Businesses that regularly ship large quantities of regulated articles out of quarantine areas need compliance agreements, an official document that specifies the conditions to be followed for growing, handling or moving regulated articles.
For compliance agreements covering logs, pulpwood and wood chips, the business prepares an "accurate statement" to accompany individual shipments. The statement includes a certifying statement, the type of regulated article, the amount or number of articles in the shipment, and the signature of the preparer.
For compliance agreements covering Christmas trees or nursery stock, shipments of regulated articles must be accompanied by a federal certificate or other authorized documentation.
Mobile Homes
For interstate movement of mobile homes from quarantined areas, a pest control operator completes the qualified certified applicator document after inspecting and treating for spongy moth. The operator must be certified by the state of Wisconsin or USDA.
Firewood
The Wisconsin Department of Agriculture, Trade and Consumer Protection certifies firewood distributors after approving a process to remove pests. Get details.
