The following farm-raised deer records must be maintained for five years and made available to the department for inspection and copying: ATCP 10.46(10) (unless otherwise noted)
For FRD that enter the herd:
The individual identifications, both official and unique, of the farm-raised deer.
Species, age and sex of the farm-raised deer.
Date on which the farm-raised deer entered the herd.
Name and address of the person from whom the person received the farm-raised deer.
Name of person who had custody of the farm- raised deer during shipment.
Address of the herd from which the farm-raised deer originated.
- Copy of a valid certificate of veterinary inspection, [ATCP 10.06(4)] and a DATCP import permit number if the deer was imported from another state (ex: 35-RB-10262-2).
For FRD that die, are killed, or are slaughtered on the herd premises:
Species, age, and sex of the FRD.
Any official identification attached to the FRD, including any carcass identification, and the number of any dead tag attached to the carcass parts.
Date on which the FRD died, was killed or slaughtered, and if the FRD was found dead on the premises, the date on which the FRD was found dead and the apparent cause of death.
The disposition of the carcass (and/or parts), including the name and address of the person who took custody of the carcass and/or the disposal method and location.
- CWD test results.
For FRD that escape:
Any identification of the FRD.
Species, age and sex of the FRD.
Date and time on which the FRD escaped the herd.
The circumstances of the escape.
The location of the escape.
Steps taken to prevent future escapes.
The date and time the FRD returned, if applicable.
The apparent number of hours the FRD was gone before returning to the herd, if applicable.
If killed, the date it was killed, date CWD sample was sent for testing, and CWD test results.
Record the report any escapes made to DATCP and DNR under section VII.
For FRD that go live to slaughter:
The official individual identification or back tag of the FRD.
Species, age and sex of the FRD.
Date on which the FRD was shipped to slaughter.
Name and address of the slaughter facility.
Name and address of the person who transported the FRD to slaughter.
Copy of the certificate of veterinary inspection, VS 1-27 or department-issued permit that accompanied the FRD to slaughter.
- CWD test results.
For live FRD that leave the herd (except for slaughter, live FRD can only leave the herd if the herd is enrolled in the CWD herd status program and meets movement requirements under section IV):
The individual identifications (at least one official and the second either official or unique to the herd) of the FRD.
Species, age and sex of the FRD.
Date on which the FRD left the herd.
Name and address of the person to whom the FRD was shipped.
Name and address of the person who transported the FRD, if different from owner.
The address of the herd to which the FRD was shipped.
- Copy of any certificate of veterinary inspection that accompanied the FRD.
For FRD moved between a single registered herd kept at 2 or more locations or separately registered herds kept at the same location (that are not medically separated):
All herds at all locations are either enrolled or all herds are not enrolled in the CWD herd status program (all herds must have the same status).
The individual identifications of the FRD (at least one official and the second either official or unique to the herd).
The species, age and sex of the FRD.
The date of movement of the FRD.
The name and address of the person who had custody of the FRD during movement, if different from owner.
- The herd registration numbers of the herd from which and to which the FRD was moved.
For known natural additions to the herd enrolled in the CWD herd status program:
The sex of the FRD, if determined.
The month and year of birth of each FRD.
- The individual identifications, both official and unique, by 12 months of age.
For known natural additions to a herd not enrolled in the CWD herd status program:
The sex of the FRD, if determined.
The month and year of birth of each FRD.
For FRD on hunting ranches. In addition to all the record keeping requirements above, for each FRD that is killed on the hunting ranch:
Name and address of the person who killed the FRD.
Date when the FRD was killed.
The lab results of any CWD tests.
All individual identification attached to or implanted in the carcass, the number of any dead tag attached to the carcass.
- The disposition of the carcass (and/or parts), including the name and address of the person who took custody of the carcass. If the carcass was buried or otherwise disposed of, the record shall identify the disposal method and location.
For FRD whose herd is enrolled in the CWD herd status program. In addition to all the record keeping requirements above, the following records must be kept for any herds enrolled in the CWD herd status program:
A record of all CWD tests conducted on FRD in the herd.
Records received from the herd veterinarian related to veterinary services provided to the herd.
