A Seafood Hazard Analysis Critical Control Point (HACCP) Training Course (4125), sponsored by the Wisconsin Department of Agriculture, Trade and Consumer Protection (WDATCP) in association with the North Central Association of Food and Drug Officials (NCAFDO) will be held November 14-16, 2017 at the Gruenhagen Conference center at the University of Wisconsin Oshkosh 208 Osceola Street Oshkosh, WI 54901.
This course is for anyone who develops, modifies or reviews Seafood HACCP Plans. The Code of Federal Regulations, 21 CFR Part 123 requires all establishments that process seafood to conduct a Hazard Analysis and if a hazard is identified, develop and follow a HACCP plan. Satisfactory completion of this course offering satisfies the training requirement indicated in 21 CFR 123.10.
Cost of this course is $100.00 per person, which covers the cost of the manuals, certification and lunch for the first two days of the course. Participants will also have to pay parking for each vehicle. Parking will be $1 a day. Please include an additional $3 for the course for parking. Please print a completed registration form and include a check or money order made payable to WDATCP with the registration. Complete a separate registration form for each person attending; submitting a single check for all registrations from one facility is acceptable. When completing the form please provide your name as you would like it to appear on your training certificate and use the address where you would like your certificate mailed following the course.

Please note that space is limited and participants will be accepted on a first come, first served basis. Confirmation of reserved seats at the course will be provided by phone or email.
The Seafood HACCP Basic Course begins promptly at 8:00 a.m. each day and will conclude around 12:00 PM on November 16, 2017. To receive your certificate you must attend the entire course, participate in the small group work and presentations, and submit a course evaluation.

The geographic boundaries of NCAFDO in the United States comprises Illinois, Indiana, Michigan, Minnesota, North Dakota, Ohio, South Dakota, and Wisconsin. In Canada, NCAFDO includes Manitoba and Saskatchewan. Membership, however, is not limited within these boundaries.
