Food and Recreational Safety Complaints

If you or someone you know is ill (or becoming ill) from a food or recreational source, call a health care facility near you immediately to receive advice or treatment.

To report a suspected foodborne illness, complete the following form maintained by the Department of Health Services: Foodborne Illness Report Form.
To report an issue on topics such as food safety, facility sanitation, employee hygiene, or third-party food delivery services, use the links below:
