If you fall within one of the categories of facilities that are required to be licensed under the law, we have three ways in which you can apply for a license:
- You can now apply and pay online using our MyDATCP site.
- You can also download and complete the application packet to the right and mail it to us. You need to sign the form, so you will need to send it via regular mail rather than email.
- Call (608) 590-8519 to have a packet mailed to you.
Those licensing as an Animal Shelter must submit a copy of their Exempt Status Certificate as proof of non-profit status. All
licensees using a business name as the legal name on their license must be
registered with Wisconsin Department of Financial Institutions (WI DFI) at
608-261-7577 or via their website.
