How to Apply
Before you apply, please complete the following steps:
- Review the Grant Application Worksheet
- Prepare the required documentation:
- W-9 tax form
- Proof of non-profit status
- Completed budget workbook
- Letters of commitment from all Wisconsin food product partners
Understand that a monthly report will be required following a grant award and that it will be required prior to any reimbursement of monthly expenses
