Before you apply, please complete the following steps:

Grant Application Worksheet
- Prepare the required documentation:
- W-9 tax form
- Proof of non-profit status (not applicable if affiliated with a Tribal Nation)
- Completed
budget workbook

that a monthly report will be required following a grant award and that it will be required prior to any reimbursement of monthly expenses
