Animals that are being sent to a Wisconsin zoo need:
Official individual identification is required for livestock species (please see species pages for details)
Required tests by species (with the exception of exotic ruminants traveling from one AZA facility to another - the tests are waived). Follow the links below for the specific requirements.
Please note: The livestock premises code for the receiving facility in Wisconsin is not required unless the animals on the permit application are from a species classified as livestock. The livestock premises code can be obtained from the owner of the destination premises.
