Reinstate Credential

​If it has been over five years since your veterinary medicine or veterinary technician credential expired, you must apply to reinstate your credential. You will need to complete and submit the following:

  • Complete an application online or by mail.
    • Online:
      • Go to MyDATCP and choose Licenses, then go to the Veterinary Examining Board section and select the credential that applies to you and click Start New Application. Complete all information and pay at the end. You will need to mail the original signed and notarized signature page-only to ATTN: VEB at the Madison address. The Veterinary Examining Board will email you with more information once your request is received and reviewed. 
    • Mail: Listed below are instructions for completing a paper application to apply by mail.
  • Pay the reinstatement fee of $185 either online after applying or mail a check to the Milwaukee lockbox address on the application.
  • Complete the social security form only if you are applying by mail.
  • Provide proof of your continuing education from the most recent two years
  • Provide letters of good standing from every state or country you have ever had a license. Active or expired.
  • Pass the Wisconsin state exam. Watch for an email (after your application has been received and processed) with instructions to register and pay to take the state exam online through applicable national testing vendors.
  • If any convictions or pending charges are checked off on the application, you will also need to complete and submit the Convictions and Pending Charges form and all required documentation.
