Restricted Use Pesticide Dealer

All retail dealers or distributors of restricted-use pesticides must be licensed. Each location that sells restricted-use pesticides must be licensed separately.

Fee: $88.00

Distributors and retail dealers of pesticides must keep sales records for two years. For record keeping requirements, see ATCP 29.15 and ATCP 29.16, Wisconsin Administrative Code.

There is an additional report that must be completed by all firms or business sites that hold a Wisconsin Restricted-Use Pesticide Dealer license. The report is due November 15 and covers the time period of
October 1 - September 30. There is no fee.

This form collects information on pesticide suppliers and all pesticide-end users. This will ensure that pesticide producers are paying proper registration fees and that pesticide distributors are properly licensed.
