2011 Unwanted Prescription Drug Collection Grant Application RFP

Applications for Prescription Drug Collections: Request for Proposals Due November 22, 2010

During the week of November 1, 2010, the department released grant application materials for prescription drug collections to be held during the calendar year of 2011. Applications are due back on or before November 22, 2010. There is $75,000 available for prescription drug collection and disposal with this Request for Proposals (RFP).

Local governments interested in a 2011 grant should download materials listed below and apply at this time.

The department expects keen competition for these grants and suggests that interested local governments carefully read the provided information after first reading the announcement letter for the grant.

2011 Unwanted Prescription Drug Collection Grant Application Materials

A completed application for a 2011 Prescription Drug Collection grant will consist of a 2011 Prescription Drug Grant Application Form, filed before the deadline, through email. Final report documents, which are due after your clean sweeps are completed, are included below.

To help applicants complete the application, we have also included a series of support files that explain the evaluation scoring system and outline controlled substance requirements. Your completed application must be received electronically by midnight, November 22, 2010 to be considered eligible for funding and returned to: DATCPCleansweep@wisconsin.gov.

Final Report Documents

If you are selected for a prescription drug collection grant, a final report is due to the department 60 days after the event.
