This tri-fold brochure provides consumers with resources that are "Free" and may prevent or reduce the risk of becoming a victim.

Law Enforcement Lobby Poster (PDF)
This poster is a great way for Law Enforcement to direct consumers to the Office of Privacy Protection who may need assistance with identity theft education and mediation.

Identity Theft Complaint Form
The complaint form is required by the Office of Privacy Protection when working with the victim to investigate or mediate their case.

Release of Information (1 page PDF)
This form is needed to request the release of personal account information to the victim in order to investigate and mediate the complaint.

General Consumer Identity Theft Presentation (22 page PDF)
This presentation gives consumers basic information to protect themselves from identity theft and fraud. It can be a helpful resource for law enforcement officers that conduct community crime prevention events.

Non-consent Form (1 page PDF)
This form is needed to confirm you are the victim of fraud to our office, the courts, creditors and law enforcement.

Victim Checklist (2 page PDF)
This checklist is for your use in preparing to file a complaint with us and a police report and to track your recovery efforts. Good records are important to a full recovery.
