Dog Breeders & Sellers Forms

These are forms  you can use to apply for a license under the dog breeder and seller law, as well as for other activities under the law. 
 
License application:  Complete packet of license application materials, including application form, instructions, and other supporting information. 
 
Variance request:  Form to request a variance from requirements of the law.
 
Temporary market: Form to notify DATCP of a temporary market selling dogs.
 
Liquidation sale: Form for breeders to notify DATCP of a one-time sale to liquidate their businesses.
